Outreach Specialist for Medication Adherence
We are seeking a dedicated and compassionate Outreach Specialist to join our team. In this role, you will be responsible for making phone calls to patients to schedule free home health visits. These visits are aimed at supporting patients with medication adherence, ensuring they receive the care and support needed to manage their health effectively.
Responsibilities
- Patient Outreach: Make outbound phone calls to patients identified for medication adherence support. Engage with patients in a friendly and professional manner to explain the benefits of free home health visits.
- Appointment Scheduling: Coordinate and schedule home health visits for patients, ensuring appointments are convenient and accessible.
- Patient Education: Provide patients with information about the importance of medication adherence and the services available to them through home health visits.
- Record Keeping: Maintain accurate and detailed records of all patient interactions, appointments scheduled, and any follow-up actions required.
- Communication: Collaborate with healthcare providers, home health teams, and other relevant stakeholders to ensure seamless care delivery.
- Follow-Up: Conduct follow-up calls to confirm appointments, address any patient concerns, and provide reminders to ensure visit compliance.
- Data Entry: Accurately input patient information and appointment details into the company’s database.
- Customer Service: Handle patient inquiries, concerns, and any issues that may arise with professionalism and empathy.
Skills
- High school diploma or equivalent; some college or healthcare-related coursework is a plus.
- Experience in a healthcare, customer service, or telemarketing role is preferred.
- Excellent communication and interpersonal skills.
- Ability to handle sensitive patient information with confidentiality and discretion.
- Strong organizational skills and attention to detail.
- Proficiency in using computers, including experience with scheduling software and databases.
