Physical Therapist (PT) - Las Cruces
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our home health care team on a PRN basis. As a PT, you will provide valuable support by developing and implementing individualized treatment plans and delivering high-quality care to patients in the comfort of their homes. Your primary responsibility will be to help patients regain mobility, strength, and independence, contributing to their overall well-being and improving their quality of life.
Responsibilities
- Assists the physician in evaluating the patient’s level of function. Assess patient’s home environment for safety hazards and architectural barriers, indentifies equipment needs.
- Identifies functional problems and establishes goals and a plan of care designed to improve the patient’s functional status (revising as necessary).
- Performs patient tests, measurements, and evaluations, such as range of motion and manual muscle tests, gait and functional analysis, and body parts measurement, and records and evaluates findings to aid in establishing or revision specifics of treatment programs.
- At the discretion of the CNO, performs comprehensive OASIS assessments, including starts of care, recertification and discharges and associated paperwork in accordance with agency policy.
- Instructs patient and family members, as well as other health team members, during certain phases of physical therapy in which they may work with the patient.
- Periodically re-evaluates and upgrades the patient’s program to maximize functional improvement.
- Administers the exercise program utilizing specialized therapeutic exercise techniques and equipment.
- Plans an administers prescribed physical therapy skilled treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury or loss of body part.
- Recommends prosthetic/orthotic devices for patients or evaluates their devices and makes minor adjustments or recommends modifications by the prosthestist/orthotist.
- Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
- Performs and submits required documentation of patient’s evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders. All patient visit notes must be turned in the next scheduled work day, not to exceed forty-eight (48) hours.
- Maintains necessary statistical and billing data.
- Orients, instructs and supervises the Home Health Aide every 14 days in cases where no skilled nursing services are being provided during the current certification period. Supervisory duties do not necessarily require accompanying the Home Health Aide during the Aide’s home visit.
- Keeps abreast of changes in physical therapy theories and practices, contributes to the ongoing development of programs and professionals within the service of sharing expertise, contributing resources and giving in-services.
- Keeps his/her immediate supervisor informed of problem areas and makes suggestions for improvement.
- Attends and contributes to in-services, clinical records reviews, meetings required to ensure coordinated and comprehensive plans of care for the patients of the agency.
- In regards to supervision of Licensed Physical Therapist Assistant assigned to carry out the Physical Therapist’s developed Plan of Care. a) The Assistant must document the name of the supervising Physical Therapist on each and every progress note; however the Physical Therapist does not have to co-sign said progress notes. b) The Physical Therapist must perform and complete a re-assessment and assistant supervisory visit every 4 weeks, and the final discharge assessment with another supervisory visit at any point thereafter within the same certification period. c) The Physical Therapist may schedule and perform supervisory visits and provide billable skilled care as frequently as the Physical Therapist deems necessary within the ordered frequency and duration set forth in the plan of care as long as the minimum is every 4 weeks. d) Supervisory visits do not require the presence of the assigned assistant in the patient’s home during said visit, however, the Physical Therapist can determine when necessary to perform on site supervisory visits with the assistant present. - The Physical Therapist will be accessible by phone and able to visit patient in the patient’s home the same day within a reasonable time frame in regards to the supervision of the treating Physical Therapy Assistant.
- Complies with Homecare Agency Compliance Plan.
- Complies with Homecare Agency’s Infection Control Policies.
- Complies with Homecare Agency’s HIPAA Policies.
Skills
- Must be a graduate of an accredited Physical Therapy program and licensed by the state of practice. Must meet the qualifications of a Physical Therapist as defined under the Federal conditions of participation for Homecare Agency Agencies established by CMS.
- The agency prefers that the Physical Therapist demonstrate at least two years of previous experience.
- Good verbal and written communication and organization skills.
- Possesses and maintains current CPR certification.
