Registered Nurse (RN)

The Home Health and Hospice Registered Nurse (RN) is responsible for planning, organizing, and directing home care services while directly reporting to the Clinical Supervisor(s). They possess extensive nursing experience with a focus on community health education and practice. The RN utilizes community resources to develop and implement services tailored to meet the needs of individuals and families within their homes and communities.

Responsibilities

  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs in their homes, including pain assessment and management, medication reconciliation, and evaluation of their support systems.
  • Develop individualized care plans in collaboration with the interdisciplinary team, including physicians, social workers, and therapists, to address patients' goals, preferences, and needs.
  • Provide skilled nursing care and interventions based on assessment findings and physician orders, including wound care, catheter care, infusion therapy, and palliative care measures.
  • Educate patients and their families on disease management, medication administration, symptom management, and end-of-life care to promote understanding and autonomy in decision-making.
  • Monitor patients' condition and response to treatment, adjusting care plans as needed and communicating changes to the interdisciplinary team and primary care providers.
  • Coordinate care transitions between healthcare settings, ensuring continuity of care and adherence to the plan of care during hospitalizations, rehabilitation stays, and hospice admissions.
  • Provide emotional support and counseling to patients and families facing serious illness, end-of-life care, and bereavement, fostering coping mechanisms and facilitating difficult conversations.
  • Collaborate with community resources and agencies to address patients' social determinants of health, such as transportation, housing, and financial assistance, to optimize their well-being.
  • Document patient assessments, interventions, and outcomes accurately and timely in electronic medical records, adhering to regulatory and agency standards for documentation and reimbursement.
  • Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to evaluate patient outcomes, identify opportunities for improvement, and enhance care delivery.
  • Adhere to ethical and legal standards of nursing practice, including maintaining patient confidentiality, respecting patients' rights, and advocating for their best interests.
  • Stay updated on evidence-based practices, clinical guidelines, and regulations related to home health and hospice care through continuing education, professional development, and participation in relevant seminars or conferences.
  • Serve as a resource and mentor for less experienced staff, providing guidance, support, and clinical expertise to promote professional growth and excellence in patient care.
  • Ensure compliance with agency policies, procedures, and regulatory requirements, including infection control protocols, HIPAA regulations, and Medicare conditions of participation.
  • Foster positive relationships with patients, families, caregivers, and healthcare partners through effective communication, empathy, and cultural sensitivity, promoting a collaborative approach to care delivery.

Skills

  • Proven experience as a registered nurse
