Home Health Care is short-term, medically directed care delivered at home. It’s designed for people who need skilled nursing or therapies and who have trouble leaving home without help.

Covered services can include wound care, medication teaching, and physical, occupational, or speech therapy when ordered by your physician. These services are meant to help the patient stabilize, learn daily self-care skills, and improve function after an illness, injury, or surgery, so that everyday life feels more manageable again.

How Home Care Starts & What to Expect

Getting started with our home health care services is simple. We verify benefits, confirm your needs, and coordinate home care services with your physician before the first in-home visit. Here’s how the home health care process works:

  1. Most families begin with a call or referral where we confirm needs and coverage.
  2. We then schedule an In-home assessment: A nurse or therapist visits to review your health history, medications, safety needs, and goals.
  3. Together with your physician, we outline a custom care plan.
  4. Clinicians visit according to the plan, coordinate with your doctor, track progress, and teach you and your caregivers what to watch for between visits.

Does Medicare cover Home Health Care?

For Medicare to cover Home Health Care, your provider must order it, you must need part-time or “intermittent” skilled services, and you must meet Medicare’s “homebound” criteria (short, infrequent outings are allowed). What insurance does Tender Care take?

For home care services, Tender Care Home Health & Hospice accepts Medicare, TriCare, VA, and most private insurances.

What if I need bilingual care?

We have several team members who are bilingual and able to provide culturally sensitive care for Spanish-speaking families.

Is home care and home health care the same?

Families sometimes confuse “home care” with “Home Health Care.” Home care usually refers to non-medical support such as bathing or housekeeping. Home Health Care is medical: nursing and therapies ordered by a physician and delivered at home, often covered by Medicare if you qualify. If you are managing a serious illness and want added relief alongside your usual treatments, Palliative Care may fit. If comfort becomes the main goal for an advanced illness, Hospice Care is the right model. We’re happy to help you understand which service meets your current needs.
