Hospice Intake Coordinator

Job Duties (include, but are not limited to the following):

• Conduct thorough intake assessments and interviews with patients and their families to determine eligibility for hospice care.
• Coordinate the admission process efficiently, overseeing all required documentation and paperwork.
• Collaborate closely with healthcare professionals, physicians, and other specialists to obtain necessary medical information and facilitate personalized care planning.
• Educate patients and their families about the benefits of hospice care and provide information about available resources.
• Maintain accurate and confidential patient records, ensuring compliance with regulatory requirements.
• Communicate effectively with our interdisciplinary hospice team to facilitate seamless patient transitions.
• Offer emotional support and compassionate guidance to patients and their families throughout the intake process.
• Participate actively in team meetings and contribute to the development of comprehensive care plans.
• Ensure strict adherence to all relevant hospice care regulations and industry standards.



  • Some schooling in healthcare administration, nursing, social work, or a related field (preferred).
  • Previous experience in healthcare intake coordination or a similar role is advantageous.
  • Exceptional communication and interpersonal skills.
  • A compassionate and empathetic approach to patient care.
  • Strong organizational and multitasking abilities.
  • Familiarity with hospice care regulations and processes is a plus.
  • Proficiency in using computer applications and electronic medical records.
